Your medical history, current symptoms, treatment plan, treatment notes including your feedback from each acutherapy session, dietary and lifestyle information, pulse and tongue diagnostics.

How we get the personal information?
The personal information we process is provided to us directly by you, by:

Covid 19 screening and consent form (in accordance with the UK government and British Association of Acupuncture (BAA) and British Acupuncture Federation (BAF) guidelines/your comfort)

Follow up pre/post treatment
Fully understand your medical history, current symptoms and priorities to provide you with the most effective treatment plan

Your consent: you have given clear consent for Sara O’Connor to process your personal data for the purpose of receiving treatment from So acutherapy. You are able to remove your consent at any time.
Legal obligation: As a qualified practitioner of acupuncture, member of the BAA and BAF and in accordance with our insurance policy.
Vital interest: the processing of your personal information is necessary to protect someone’s life.
How we store your personal information?
Your information is securely stored in a secure, locked filing cabinet accessed only by Sara O’Connor.
The British Acupuncture Federation Code of Professional Conduct requires registrants to keep client records for a minimum of seven years. In the case of minors, records must be kept until the patient reaches the age of twenty-five (seven years after reaching eighteen). This applies even when So acutherapy has referred the patient on or have left the practice where treatment was carried out.
We will then dispose of your information by shredding all paper documents and deleting all email correspondence including your email address from So acutherapy’s contacts.
